Grayson’s Gift, Inc. Karate Scholarship Information

Exhibit “A”

 

Dear Applicant,

 

Thank you for your interest in the Karate Scholarship Program being offered by Grayson’s Gift, Inc.  a limited number of students will be awarded a scholarship to attend the Adapted Karate Classes offered by Keith Mathews Kenpo Karate that will be valid until December 31st, 2008.

 

The following stipulations are understood and agreed to by the applicant for this funding:

 

· The parent or guardian arranges transportation to and from class.

· One parent or guardian must accompany the child to the class and remain while class is in session.

· Class is scheduled for the 1st and 3rd Saturday of each month from 2:30pm to 3:15pm.  Occasionally classes can be rescheduled by Keith Mathews, the Instructor.  Students will be notified of any changes.

· Class takes place at facilities offered by First Christian Church, 1270 Sawnee Dr, Cumming, GA.

· Scholarship will be awarded for the period ending December 31st, 2008.

· Funds awarded will go directly to Keith Mathews Kenpo Karate, and applied for students who have received the scholarship. (Kenpo Karate 678-493-3810)

· Parent and student must be committed to attending classes regularly.  If special circumstances arise and the student can no longer attend classes, Grayson’s Gift, Inc. must be notified immediately so that the scholarship may be transferred.

· Parent will be responsible for filling out applicable forms provided by Keith Mathews Kenpo Karate for participation in the class.

By signing the application and waiver form, you are agreeing to these terms.

 

Grayson’s Gift, Inc. shall have no liability, whatsoever for injury to person or property as a result of the participation in the Adapted Karate Class.  Grayson’s Gift, Inc. shall merely provide funding assistance, if approved, to the Applicant.
